Through the Covidien fund, your program may be eligible to receive a visit from an FLS proctor to administer the exam to all of your eligible upper level general surgery residents and any surgical attendings with a valid test voucher. Up to 12 test candidates can be tested in one day. Eligible programs must meet the following requirements to receive onsite testing through the Covidien fund:
- Your program must be at least 60 miles from an FLS Test Center. As a reminder, valid test vouchers may be used at any FLS Test Center, the ACS Congress or the SAGES Annual Meeting. For a list of all FLS Test Centers in the U.S. and Canada, please click on the testing information tab at the top of the page.
- At least six (6) general surgery residents must be scheduled to take the exam on the requested date (up to a maximum of 12 test candidates per day). If your residency program has more than 12 test candidates, you may be eligible for a second day of testing. Please contact Inga Brissman at inga@flsprogram.org for more information.
- Your program must submit a completed request form at least two (2) months prior to the requested date(s) in order to be considered for onsite testing
Programs may be eligible for up to three (3) sponsored onsite testing visits during the entire duration of the Covidien fund (April 2008-December 2013).
