There are four FLS Packages to choose from
Choose the Individual Package or Education Packages A, B or C depending on how many candidates will be participating in the FLS Program. Each Test Voucher allows you to take the FLS Test at:
Individual Package
Geared for individual residents, fellows and practicing surgeons. FLS Exam taken at any Test Center, SAGES Annual Meeting or ACS Clinical Congress
- 1 CD-ROM set (includes CD No. 1 Didactic and CD No. 2 Manual Skills)
- 1 Numbered testing voucher
- Test Center list with contact information
- Program information, where to test, and how you receive your Certification of Completion
- Trainer Box order information
Education Package A
Geared for smaller residency programs, MIS skills labs and medical staff offices. FLS Exam taken at Test Centers, SAGES Annual Meeting and ACS Clinical Congress
- 3 Master CD-ROM set (includes CD No. 1 Didactic and CD No. 2 Manual Skills)
- 3 Numbered testing vouchers (Additional testing vouchers can be ordered)
- Test Center list with contact information
- Program information, where to test, and how you receive your Certification of Completion
- Trainer Box order information
Education Package B
Geared for residency programs, MIS skills labs, surgery departments and privileging committees. FLS Exam taken at Test Centers (5+ individuals), SAGES Annual Meeting and ACS Clinical Congress.
- 5 Master CD-ROM sets
- 5 Numbered testing vouchers with group name on each allowing candidates to take the exam at any Test Center (Additional testing vouchers can be ordered)
- Test Center list with contact information and available testing dates for the year
- Program information, where to test, and how you receive your Certification of Completion
- Trainer Box included
- Trainer Box Accessory Kit recommended
Education Package C
Geared for residency programs, skills labs, surgery departments and privileging committees. FLS Exam proctored On-Sites (5+ individuals)
- 5 Master CD-ROM sets
- 5 Numbered testing vouchers with group name on each (Additional testing vouchers can be ordered)
- Program information, how you receive your Certification of Completion and how to arrange on-site testing and proctor
- An FLS Proctor travels to your institution for a specified number of days to administer the FLS Exam. Maximum of 10 tests can be given per day. FLS program covers cost of proctor's airfare. Site is responsible for hotel and per diem.
