Institutions with a large number of eligible test takers can elect to schedule a special ‘traveling proctor’ to administer the FLS exam at their site for a fee. Please note a minimum of 8 test takers is required to schedule onsite testing. All eligible test-takers must possess a valid FLS test voucher (vouchers are not included in the onsite testing fee). Please fill out a request form using the link below to request a date for onsite testing at your facility. Dates for onsite testing must be requested at least 2 months in advance. We will make every effort to accommodate one of your three requested dates as indicated on the form. Once we are able to confirm a requested date for onsite testing at your facility, we will send you a confirmation email, along with an invoice for onsite testing. Payment for onsite testing must be received at least 30 days prior to the confirmed date. Your confirmation email will include pertinent information for preparation of an FLS Onsite Testing event (including the information that must be submitted to the SAGES office prior to the event).
The traveling proctor will provide the consumable materials for the event (suture, endoloops, gauze, penrose drains, foam organs). The facility is responsible for providing all other required equipment and supplies (trainer box, monitor, computers, etc). For more information regarding the required FLS materials, please go to:
The price for 1 full day of FLS Onsite testing is $2550 for institutions located more than 60 miles from an FLS Test Center, and $3550 for institutions within 60 miles of a Test Center (includes consumable testing materials). Please see the map of FLS Test Centers below to verify if your institution is within 60 miles of an FLS Test Center. Up to 12 test takers can be accommodated in one day (a minimum of 8 is required). Institutions with more than 12 test takers can schedule an additional consecutive day of testing at $1000 for each consecutive onsite testing day. FLS Onsite Testing is currently only available to institutions in the U.S. and Canada
